Updated Student Information Form

Students should fill this form out if any of their information has changed recently. It is recommended that every student completes this at the beginning of each school year. Student's phone number, email, and mailing address should be up to date at all times.

All students are required to complete a medical form if Sinclair Upward Bound does not have one on file. These are mandatory for field trips.
